OnD Connect is a ‘real-time’ monitoring platform giving patients, caregivers and healthcare professional the tools to reduce the rate of in-patient care and hospital readmissions caused by poor patient self-management.

How does it Work:

Launched May of 2013, our OnD ‘Bridge’ device currently records and transmits ‘real-time’ medical data from thousands of patients throughout the states each year.  The Bridge connects to a host of recording devices including our one-touch GlucoLink glucose meter.  Blood glucose readings are then transmitted in real-time to our secure servers for processing. 


Once received, trend data on each patient is analyzed continuously for variations from the patients set limits.  Abnormal variations post back to the patient’s Bridge device allowing them to see interactive messages and take action.  The OnD Bridge has an easy to read color touch screen to facilitate patient ease of use.


ADDITIONAL FEATURES ALLOW THE PATIENT’S PHYSICIAN AND CAREGIVES TO BE ENGAGED

 

For caregivers including concerned family members, OnD Connect allows transmission of SMS and/or email alerts keeping them informed of trends which might result in the need for additional care. 


Physicians have access to their patients via the OnD ‘Control Panel’.  This web based system can be accessed via p.c., smart phone or any tablet device.  Physicians can either login automatically after having received an email regarding abnormal trends in their patient’s condition or login at random to monitor their patient base as a whole.

The OnD Control Panel is customizable and interactive allowing physicians to send notifications back to their patients via email, SMS or request the platform to instruct the physcian’s staff to follow-up live with the patient.

Is a Bridge device required?

No. Patients of all ages already have a variety of smart phones and tablet devices. For some carrying an extra device might not be a convenient alternative which may affect a patient's compliance. OnD Connect has the flexibility to allow patients to download our app and perform a one-time 'pairing' function with our glucose monitor to get started. Once paired, patients receive the same notifications and trend data as with our Bridge device.



Is This System Reliable and Compliant?

OnD’s Real-Time oximetry platform has undergone extensive testing throughout the nation. Our platform transmits to our secure server using unique device identification numbers and twice the level of encryption coding required by HIPAA.
